For our movie review project I choose the movie Patch Adams. It was a movie I had seen a couple times when I was younger that I knew would have different meaning after taking this course. Based on the true-life events of Hunter Dorerty 'Patch' Adams, we follow Patch as he tries to find his place in the medical world. After self-committing himself into a mental institution, Patch Adams discovers a passion for helping his fellow inmates. Through his experiences at the mental hospital, Patch sets his sights on becoming a doctor. Once arriving at medical school he is in disbelief to find that he is not supposed to have contact with patients until his third year. This does not stop him, by bringing laughter and joy, Patch is able to breathe life back into the sick patients at the hospital. His unorthodox method of healing constantly gets him into trouble, regardless of the fact that he has some of the highest grades in his class. Inspired by the patients, Patch opens the Gesundheit Institute, a free clinic run by himself and a few of his classmates. When the Dean of the college discovers Patch is practicing medicine without a license, he has enough ammunition to keep Patch from graduating. In front of the Medical Review Board, Patch must make his case to prove he should be a doctor. While unconventional and overly involved, Patch reminds us that, 'Our job is improving the quality of life, not just delaying death.'
In week two of our course we found the first theme connecting to Patch Adams. During this week, we learned about health through ethnography. Illness narratives and explanatory models were used to explain the complex doctor-patient relationship. In the book Illness Narratives by Arthur Kleinman, there is a section that follows a conversation between a patient and her new doctor. The woman has had psoriasis for fifteen years and thus so, became well versed in the subject. During their initial meeting the doctor asks some general questions regarding the biomedical history of her condition,
'Dr. Jones: where did it begin?
Mrs. Lawler: I was in college, under lots of pressure from exams, and there is a family history of skin problems. It was winter and I was wearing heavy woolen sweaters that seemed to bother my skin. My diet was–
Dr. Jones: No, No! I meant where on your skin did you first notice plaques?
Mrs. Lawler: my shoulders and knees. But I had a problem for some time with my scalp I never—
Dr. Jones: how has it progressed the past few years
Mrs. Lawler: these have been difficult years, I mean I have been under great stresses at work and n my personal life I—
Dr. Jones: I meant, how has your skin problems progressed?' (Kleinman 1998, 129)
In this encounter we have a woman who believes there is an association between life stress and her illness, speaking to a doctor whom is only viewing her illness as a medical condition. In Patch Adams, we see a very similar theme. From the start, when Patch is in the mental health facility, the on-call psychologist has a complete lack of interest in having any sort of personal relationship with Patch or any patient. This arms-length philosophy inspires Patch to focus more on a patient's emotional needs and quality of life than their disease state.

In week four of our course we learned about the medicalization of human life. In 1910 The Flexner Report came out, stating that biomedicine was superior to any other kind of medicine. Flexner demanded that all medical schools stop teaching anything not based on biological or chemical sciences. This ideology has drastically impacted the practice of physicians in the United States today. Patch believed in a different kind of medicine, he believed in the power of laughter and compassion. During lecture 4.1 the subject of the medicalization of death was discussed. Until recent history, in the United States, death was always considered a normal part of life. Dying is one of the only universal constants in our world. In the movie, Patch makes it very clear how he feels about death, 'We're all dying, our job is to improve health'. He believed in increasing the quality of life not attempting to prevent death and that death could be treated with humility and humor. During his hearing before the Medical Review Board, he informs the board that when you 'treat a illness you win or lose, treat a patient you'll always win'.
I truly believe that anyone going into the medical field should watch this movie. For most of our education we are taught to treat the body as a machine. We are reminded that the body and the mind are separate entities, and that medications and surgery are the only worthwhile ways to treat an illness. This class in general has been extremely thought provoking. It has changed the way I view medicine and the way I will someday practice it. Patch Adams would fit very nicely into the curriculum because not only is it based on a true story, but also offers an entertainingly important message that a patient is a person and there is nothing wrong with being excessively happy.
